[raster] Rewrite ST_CreateIndexRaster as a wrapper around ST_MapAlgebraExpr
Now that we can refer to the x and y of the pixel being computed, we can rewrite the ST_CreateIndexRaster() prototype as a wrapper around ST_MapalgebraExpr(). Should become 100 time faster...

not sure if this should be pushed to 2.0.3 or 2.1.0 or how relevant it is giving all the rewriting of ST_MapAlgebra plumbing. I think its too late for 2.0.2
